Winchester Older Peoples Community Innovations Service
The Community Innovations initiative is a joint scheme between Hampshire County Council, Health, district councils and the voluntary sector.
The Community Innovations service provides support, advice and guidance to older people who are not currently in crisis, but beginning to experience difficulties around the home; or in managing everyday activities; or having concerns about health and wellbeing.
How does it work?
The teams are different in each geographical area. Our local team in Winchester consists of the following:
- A Social Worker a Occupational Therapist and Support Worker (based at Adult Services) who has knowledge and expertise in health and social care.
- A Early Interventions Nurse (based within Hampshire PCT) who has a broad knowledge in health and medical matters
- A Finance Advice Worker (Winchester Citizens Advice)
- A Community Development Worker who supports volunteer run groups and activities (Winchester Area Community Action). Who has a good knowledge of what is available to you in your local area. How can we help?
The Community Innovations Team provide assistance in two ways
Individuals
- By providing support, advice and guidance on a range of issues such as keeping warm, safe, fit and eating well .
- Providing specialist help if you are experiencing difficulties around the home, or with managing everyday activities, or if you are concerned about your health and wellbeing.
- Providing assistance with linking you into appropriate health and community services. The wider community
The Community Innovations Development Worker (CIDW) is able to assist Voluntary groups in a number of ways which could include:
- Assist communities who are exploring ways to provide a local resource for older people in their community.
- Offering support, advice and guidance to voluntary sector groups and organisations that provide support, advice and or activities which include older people in the community. (This will include a wide range of groups including special interest groups and not just groups for older people).
- Engage with existing groups and local people who are interested in setting up or expanding activities and services for older people in the area
- Provide support and assistance when required to strengthen existing networks and support networking, and assist in sharing of experiences and best practice between groups
- Assist local groups and communities to develop their marketing and publicity skills to ensure that local people know what is available in their area
- Assisting individuals to take part in activities they are interested in.
- The CIDW is based in The Winchester Centre as part of the development team she works 18 hours per week Monday to Wednesday
